Functional Capacity Assessment

Our experienced Occupational Therapists conduct in depth Functional Assessments that can be utilised in a variety of settings and circumstances. These assessments evaluate functional abilities, barriers and support requirements that can help guide implementation of supports and ensure appropriate funding is allocated.

Our team has several years experience in the NDIS sector and pride themselves on providing comprehensive, high quality reports to advocate for each participants needs. Our Functional Assessment and reports can support plan reviews, change of circumstance requests, NDIS access requests and Administrative Review Tribunal (ART) matters to name a few.

Assistive Technology Assessment

Our Occupational Therapists have extensive experience and knowledge in Assistive Technology assessment and prescription. We have experience with the following Assistive Technology:

    Our recommendations are made following a comprehensive assessment, goal setting and equipment trials. Where required, we provide comprehensive reports with strong clinical reasoning to support in advocating for and securing the appropriate funding.

    A critical part of our service is ensuring that adequate training and follow-up are provided to participants and their caregivers post-delivery. This is aimed at instilling confidence, ensuring appropriate implementation, and ensuring that goals are achieved as intended.

Home and Living Assessment

Our Home and Living Assessments help identify safe, appropriate and sustainable living options, including but not limited to Specialised Disability Accommodation (SDA), Supported Independent Living (SIL), Independent Living Options (ILO). We consider your goals and preferences, daily living needs, environment and support requirements to develop tailored recommendations for independent and supported living arrangements.

Our team has extensive experience with successful Home and Living applications, and will continue to advocate for the right living arrangement to achieve a better quality of living and independence. Our team are also passionate about supporting younger adults with disabilities to transition out of Residential Aged Care and into appropriate supported accommodation that better aligns with their goals and support needs.

Home Modification Assessment

We assess, plan and recommend home modifications to improve safety, accessibility and independence. Our Occupational Therapists provide assessments and detailed reports to support functional needs and create safer, more accessible living environments that are compliant with the Australian Standards where possible.

Whether it is minor modifications such as grabrail and hand-held shower hose installation, or complex modifications such as an accessible bathroom modification or access ramp — our team will work alongside you and a suitable builder to develop a comprehensive scope of works and secure funding where applicable.

NDIS Access Reports

Following a comprehensive assessment, our Occupational Therapists prepare detailed NDIS Access reports that clearly outline the impact of your condition on everyday life, and highlight recommendations to help strengthen your application and support your NDIS access request.

Vehicle Modification Assessment

Our Occupational Therapy team provide assessments for vehicle modifications that support safe and accessible transport. This includes evaluating mobility, transfer abilities, seating needs, and existing equipment to recommend suitable modifications for adults and children, such as:

  • Vehicle access solutions

  • Transfer aids

  • Wheelchair hoists and loaders

  • Passenger seating adjustments

  • Anchor points and restraint systems

  • Storage solutions for mobility equipment

A detailed OT report is provided to help secure appropriate funding and guide installation. Following installation, our team ensure appropriate follow-up to assist with implementation and ensure goals are achieved as intended.

Please note: we do not provide assessments for driving-related vehicle modifications (e.g., hand controls, driving controls, or adaptive driving systems). If these are required, we will arrange a referral to a partnering driver-trained Occupational Therapist for specialist assessment.

MACA Certified Therapists

Our team are MACA-Certified, which means that we are trained in best-practice vehicle safety for children and adults with disabilities.

Being MACA-certified means that we follow nationally endorsed guidelines for safe transport, including the selection and use of specialised restraint systems, seating supports, harnesses, and equipment designed for passengers with complex positioning or behavioural needs.

Ongoing Therapy & Capacity Building

We work alongside participants to build practical life skills for everyday living — including but not limited to:

  • Self Care

  • Domestic Tasks

  • Vocational Activities

  • Sport and Recreation

  • Community and Social Participation

    Whilst we offer assessment services to all age groups, we currently only offer therapy services to participants 15 years and older. If you know someone younger than 15 who is in need of ongoing therapy, we are happy to provide recommendations for other fantastic Occupational Therapists in our network.

Aged Care Support

Our aged care Occupational Therapy services support older adults to remain safe, independent and active in their own homes for as long as possible. Through My Aged Care, we deliver personalised assessments and practical strategies to enhance everyday living and community participation.
Some examples of our support include:

  • Equipment prescription (eg: profiling beds, pressure care mattress, wheelchairs, lift recliner chairs, bathroom aids)

  • Home modifications (eg: bathroom, access ramps, grabrails)

  • Pressure Care Management

  • Personal Care Independence

  • Medication Management

  • Meal Preparation Skills
